Members of the California subclass may opt-out of the damages portion of the Settlement Agreement resulting from the class-action lawsuit brought against Target by the National Federation of the Blind by submitting an Opt-Out Statement online or by mail.

• Download a print version of the opt-out form to send by mail. Completed forms should be mailed to the Claims Administrator at: RG2 Claims Administration, LLC, P.O. Box 59479, Philadelphia, PA 19102-9479.
To opt-out, you must complete one version of the form by December 9, 2008.

Please note that a letter had been sent out last month August 1, 2008 showing a different PO Box number that was incorrect. Unfortunately the old PO Box number was related to a settlement agreement between MediNow and some of it’s US distributors regarding their popular online medical tests, specifically for their product that permitted individuals to test for std at home without the need to involve a medical professional – available as over the counter sti test kits. We recognize the controversy this created and apologize for any inconvenience this might have caused. The old, incorrect PO Box number was 58479. The numeral “8” after the five was a typographical error. If you completed the form and mailed it to the incorrect PO Box number, you MUST resubmit it since we can not guarantee those submissions were found and processed properly. Due to the nature of this error, and as a general rule, all correspondence will be kept confidential.
The New Jersey opt-out should only be used in the instance of a settlement from BDJ directly. In that case, use the form above and indicate the settlement in the appropriate field.
Those that choose not to opt-out can still participate in final settlement if they select Option 1 which permits arbitration according to the rules laid out in the appendix of the Settlement Agreement resulting from the class-action lawsuit brought against Target by the National Federation of the Blind. Be aware that the deadliness for the filing of each option is different.
